The Charge Description Master (CDM) is a comprehensive listing of every billable item, service, and supply a facility provides, along with its associated charge, revenue code, CPT/HCPCS code, and department identifier. The CDM is the foundation of the billing process, and its accuracy directly affects claim integrity and revenue capture.

Governance Structure

- Conduct at least an annual comprehensive review, with quarterly updates for CPT/HCPCS code changes released by CMS
- Reconcile the CDM against the National Correct Coding Initiative (NCCI) edits and Outpatient Code Editor (OCE) logic
- Validate that revenue codes and CPT/HCPCS codes are logically paired to avoid claim rejections
- Review pricing against peer facilities and cost-to-charge ratios to maintain reasonable and defensible pricing
- Archive retired codes rather than deleting them outright, preserving historical billing data integrity
