Clinical decision support, or CDS, refers to tools and systems that provide clinicians, staff, and patients with knowledge and person-specific information, intelligently filtered and presented at appropriate times, to enhance health and healthcare decisions. CDS is a foundational component of modern electronic health record systems.
Types of Clinical Decision Support
CDS tools include drug interaction and allergy alerts, reminders for preventive care such as screenings and immunizations, order sets that bundle evidence-based orders for specific conditions, diagnostic support tools, and dashboards presenting relevant patient data at the point of care. Passive CDS provides information on request, while active CDS interrupts workflow with alerts requiring acknowledgment.
Alert Fatigue
Alert fatigue occurs when clinicians are exposed to excessive numbers of CDS alerts, many of which are low-value or easily dismissed, leading to desensitization and increased risk of overriding even clinically significant warnings. Alert fatigue is a well-documented patient safety concern that organizations must actively manage through careful alert design and tiering.
Evidence-Based Rules
Effective CDS rules are grounded in current clinical evidence and guidelines, regularly reviewed and updated as medical knowledge evolves. The five rights framework holds that effective CDS delivers the right information, to the right person, in the right format, through the right channel, at the right point in workflow. Successful CDS implementation involves clinical stakeholder input during design, careful alert prioritization to reserve interruptive alerts for high-severity situations, ongoing monitoring of alert override rates, and periodic governance review to retire low-value rules. HIM professionals contribute by ensuring CDS logic correctly references structured data fields and supports accurate downstream documentation and coding.
