Why Standardized Terminology Matters

Standardized clinical terminologies enable consistent, computable representation of health data across different systems and organizations. Without them, semantic interoperability would be impossible, and data exchanged between systems could be misinterpreted.

SNOMED CT Structure

Systematized Nomenclature of Medicine Clinical Terms (SNOMED CT) is a comprehensive, multilingual clinical terminology used to code diagnoses, findings, procedures, and other clinical concepts. It is organized as a hierarchical structure of concepts, each with a unique identifier, descriptions, and relationships to other concepts, allowing for detailed and precise clinical documentation.

LOINC Purpose

Logical Observation Identifiers Names and Codes (LOINC) is a terminology standard specifically designed for identifying laboratory tests, clinical observations, and measurements. LOINC codes standardize the naming of tests and results, such as blood glucose or hemoglobin A1c, so results from different laboratories can be compared and aggregated accurately.

Mapping Between Terminologies

Because different systems may use different terminologies, cross-mapping is essential. Organizations often map local codes to SNOMED CT or LOINC, and map SNOMED CT concepts to classification systems like ICD-10-CM for statistical and billing purposes.

Use in EHRs

Certified EHR systems are required to use SNOMED CT for problem lists and LOINC for laboratory test ordering and results reporting under federal certification criteria. This ensures data captured at the point of care can be exchanged meaningfully with other systems, including public health reporting systems.
