Balancing Speed and Accuracy

Coding departments must manage two competing priorities: coding claims quickly enough to support timely billing and cash flow, and coding accurately enough to withstand audit scrutiny. RHIA candidates should understand how productivity and quality standards are developed and monitored together.

Productivity Standards

  • Typically expressed as charts coded per hour or per day, varying by record type (inpatient, same-day surgery, emergency department, ancillary)
  • Benchmarks often come from industry organizations such as AHIMA, adjusted for organizational complexity and EHR/CAC tool efficiency
  • Productivity expectations for complex inpatient records are lower than for straightforward outpatient ancillary charges

Quality Standards

Most coding departments target a minimum accuracy rate, commonly 95 percent or higher, measured through regular internal quality audits comparing coder assignments against documentation and official coding guidelines. Accuracy is typically measured at both the code level and the DRG/APC assignment level.

Balancing the Two

  1. Set productivity standards only after establishing a baseline accuracy expectation, never rewarding speed at the expense of quality
  2. Use a graduated productivity ramp-up for new coders while accuracy is closely monitored
  3. Provide real-time feedback through quality audits rather than waiting for annual performance reviews
  4. Adjust productivity standards when CAC tools, EHR changes, or new coding guidelines affect coding complexity

Computer-Assisted Coding's Role

CAC technology uses natural language processing to suggest codes based on clinical documentation, which coders then validate and finalize. CAC can improve productivity, but human coder validation remains essential because CAC suggestions can miss context, over-code, or under-code without clinical judgment.

Performance Management

Coding managers typically track individual coder productivity and accuracy trends over time, using the data to identify training needs, recognize high performers, and address performance issues through structured improvement plans rather than punitive action alone.

Be prepared to explain why accuracy standards should be established before productivity standards, and how CAC technology changes, but does not eliminate, the coder's role in the coding process.
