What Is a Cohort Study?
A cohort study follows a defined group of individuals, called a cohort, over time to observe the development of an outcome, such as a disease or complication, in relation to an exposure of interest. RHIA candidates should understand cohort studies as a foundational epidemiological design used heavily in population health analytics and outcomes research.
Prospective Versus Retrospective Cohorts
A prospective cohort study identifies a group and follows them forward in time, collecting data as outcomes occur, which allows for careful control over data collection but requires significant time and resources. A retrospective cohort study, by contrast, uses existing records to look backward, identifying an exposed and unexposed group at a point in the past and tracking outcomes that have already occurred, which is faster and less costly but depends heavily on the completeness and accuracy of historical documentation.
Key Measures in Cohort Studies
Cohort studies commonly calculate incidence rates, comparing the rate of new cases in the exposed group to the unexposed group, and relative risk, which expresses how many times more likely the exposed group is to develop the outcome compared to the unexposed group. A relative risk greater than one suggests increased risk associated with the exposure.

Strengths and Limitations
Cohort studies can establish temporal relationships between exposure and outcome, which is a strength over cross-sectional designs, but they can be affected by loss to follow-up, confounding variables, and, in retrospective designs, incomplete or inconsistent historical records.
Application in Quality Improvement
Hospitals often use cohort methodology internally, comparing outcomes for patients exposed to a new clinical protocol against a historical or concurrent comparison group, to evaluate whether the protocol improved outcomes such as infection rates or length of stay.

Be able to distinguish cohort studies from case-control studies, remembering that cohort studies start with exposure status and move forward toward outcomes, while case-control studies start with outcome status and look backward toward exposures.
