Interoperability Depends on Governance

Interoperability, the ability of different health IT systems to exchange and meaningfully use data, depends heavily on strong data governance. Without governed data definitions, terminology mappings, and quality controls, exchanged data may be technically transmitted but clinically unusable. RHIA candidates should understand how governance strategies specifically support interoperability goals.

Levels of Interoperability

  • Foundational: Basic ability for one system to send data to another without requiring the receiving system to interpret it.
  • Structural: Data is exchanged in a standardized format so the receiving system can parse it into discrete fields.
  • Semantic: Data is exchanged with standardized coded terminology, allowing the receiving system to interpret the clinical meaning consistently.
  • Organizational: Governance, policy, and workflow alignment across organizations enabling secure, consistent, and timely data exchange at scale.

Governance for Semantic Interoperability

Semantic interoperability requires governed terminology mapping, ensuring local codes are consistently mapped to standard terminologies like SNOMED CT, LOINC, or RxNorm. A governance committee must approve and periodically review these mappings, since incorrect mappings can propagate clinical errors across every organization receiving the data.

Data Quality Firewalls

Some organizations implement data quality validation rules at the point of exchange, rejecting or flagging messages that fail completeness or format checks before they reach a receiving system. This "firewall" approach prevents poor-quality data from propagating across a health information exchange network.

Trust Frameworks and Data Use Agreements

Organizational interoperability also depends on trust frameworks and data use agreements that define how participating organizations will handle, secure, and govern shared data. These agreements typically specify permitted uses, security requirements, and dispute resolution procedures.

  1. Standardize terminology mappings and subject them to governance review.
  2. Implement data quality validation at exchange points.
  3. Establish data use agreements defining shared governance expectations.
  4. Monitor exchange performance metrics, such as message rejection rates.
