Defining Data Lineage and Provenance

Data lineage refers to the documented path data takes as it moves and transforms across systems, from its original source to its final destination. Data provenance refers specifically to the origin and history of a data element, including who created, modified, or transmitted it. Together, these concepts allow organizations to trace data back to its source and verify its trustworthiness, a concept tested on the RHIA exam within data governance and quality contexts.

Why Lineage Matters in Healthcare

Provenance in the Legal Health Record

Within an EHR, provenance metadata captures the author, timestamp, and any amendments to clinical documentation. This is essential for distinguishing an original entry from a later addendum or correction, which has significant implications for both patient care and legal proceedings. HIM professionals must understand how their EHR system displays and preserves this provenance information.

Governance Responsibilities

Data governance committees should require lineage documentation for any data element used in external reporting or quality measures, and they should periodically audit that lineage documentation remains accurate as systems and interfaces change.
