Why Data Standardization Matters
Health data comes from many sources, including different electronic health record systems, laboratories, and billing platforms, each of which may represent the same information differently. Data normalization and standardization ensure that data can be reliably combined, compared, and analyzed across these sources, an essential concept for the RHIA exam.
Database Normalization
In database design, normalization refers to organizing data to reduce redundancy and improve integrity. This is typically described through normal forms.
- First normal form eliminates repeating groups and ensures each field contains a single value.
- Second normal form removes partial dependencies, ensuring non-key fields depend on the entire primary key.
- Third normal form removes transitive dependencies, ensuring non-key fields depend only on the primary key.
Terminology Standardization
Beyond database structure, clinical data must be standardized using controlled vocabularies so that the same clinical concept is represented consistently across systems. Common standards include SNOMED CT for clinical terms, LOINC for laboratory and clinical observations, RxNorm for medications, and ICD-10-CM and CPT for diagnosis and procedure coding.
Why Standardization Enables Analytics

Different facilities may also define the same metric differently, such as calculating length of stay using different rounding rules, leading to inconsistent benchmarking results.
Master Data Management
Master data management establishes a single, authoritative source for key data elements, such as patient identity through a master patient index or provider identity through a master provider index. This reduces duplicate records and supports more accurate longitudinal analytics across an organization or health information exchange.
Role of HIM Professionals
Health information managers often lead or participate in data governance committees responsible for defining standard data elements, resolving mapping discrepancies, and auditing data quality to ensure standardization efforts are maintained over time.
