Data provenance refers to documenting the origin, history, and chain of custody of a piece of data, including who created it, when, and how it may have been modified. Provenance in Health Information Exchange

Without proper provenance metadata, it becomes difficult to determine which organization is the authoritative source and whether the data has been altered in transit.

Copy-forward and copy-paste practices in EHRs pose specific provenance risks. If a clinician copies outdated information forward without updating it, the record may misrepresent the patient's current condition.
