Why Stewardship Structures Matter
Effective data governance depends on clearly defined roles. Without accountability, data quality issues go unaddressed and no one is responsible for resolving conflicting definitions across departments. The RHIA exam tests knowledge of these distinct roles within a governance framework.
Data Owners
Data owners are typically senior leaders or department heads who have ultimate accountability for a specific data domain, such as the CMIO for clinical data or the CFO for financial data. Owners approve data definitions, access policies, and quality standards for their domain.
Data Stewards
Data stewards are subject matter experts who manage day-to-day data quality within their domain. HIM professionals frequently serve as data stewards for demographic, diagnostic, and procedural data. Stewards resolve data quality issues, maintain data dictionaries, and ensure consistent use of terminology across systems.
Data Custodians
Data custodians are typically IT staff responsible for the technical storage, security, and maintenance of data systems. Custodians implement the access controls and backup procedures that owners and stewards require, but they do not make policy decisions about how data is used.
Governance Committees
A data governance council or committee typically includes representatives from HIM, IT, compliance, clinical departments, and executive leadership. This body sets enterprise-wide policy, resolves cross-departmental disputes, and prioritizes data quality initiatives.
Accountability Frameworks
A RACI matrix (Responsible, Accountable, Consulted, Informed) is a common tool for clarifying stewardship roles on specific projects. Documenting these relationships prevents duplication of effort and ensures gaps in accountability are identified before they cause data integrity problems.
