EHR Interoperability Standards Every RHIA Candidate Should Know

The Interoperability Challenge

Interoperability is the ability of different health information systems to exchange, interpret, and use data cohesively. The RHIA exam expects candidates to recognize the major standards that make this possible and to understand the levels of interoperability defined by HIMSS: foundational, structural, semantic, and organizational.

  • HL7 v2: the long standing messaging standard used for admission, discharge, transfer, and order communication between systems.
  • HL7 FHIR: Fast Healthcare Interoperability Resources, a modern, web based standard using discrete resources and RESTful APIs that has become the foundation for app based data exchange and the standard referenced in current federal interoperability rules.
  • CDA and C-CDA: Clinical Document Architecture and Consolidated CDA, used to structure documents such as continuity of care documents shared during transitions of care.
  • DICOM: the standard for storing and transmitting medical images.

Semantic Interoperability

Structural standards alone do not guarantee that two systems interpret data the same way. Semantic interoperability requires standardized vocabularies such as SNOMED CT for clinical terms, LOINC for laboratory and observation data, and RxNorm for medications. Candidates should be able to match each vocabulary to its primary use case, since exam items frequently ask which terminology is appropriate for a given data element.

Regulatory Drivers

The 21st Century Cures Act and its Information Blocking provisions, along with ONC's Health IT Certification Program, require certified EHR technology to support FHIR based APIs so patients and providers can access electronic health information without unreasonable restriction. Understanding information blocking exceptions, such as the preventing harm exception or the infeasibility exception, is a common exam topic.

Health Information Exchange

Health Information Exchanges, or HIEs, rely on these standards to move data between unaffiliated organizations. HIM professionals must understand consent models for HIE participation, including opt in and opt out approaches, and how patient matching accuracy directly affects the reliability of exchanged data.

When a question describes an app pulling structured lab results from multiple hospital systems, the expected answer involves FHIR APIs and standardized terminology, not older messaging formats. When a question focuses on document based transitions of care summaries, C-CDA is typically the right answer.
