Physical Safeguards Defined
Physical safeguards under the HIPAA Security Rule address the physical measures, policies, and procedures used to protect electronic information systems and related buildings and equipment from natural and environmental hazards and unauthorized intrusion. These safeguards complement administrative and technical controls to provide comprehensive protection of ePHI.
Facility Access Controls
Facility access control standards require policies limiting physical access to facilities housing information systems while ensuring properly authorized access is allowed. This includes contingency operations procedures for restoring data access during an emergency, facility security plans to safeguard the facility from unauthorized physical access, access control and validation procedures such as badge systems, and maintenance records documenting repairs and modifications to physical security components.
Workstation Use and Security
Workstation use policies specify the proper functions to be performed and the manner in which they should be performed on workstations that access ePHI. Workstation security standards require physical safeguards for all workstations that access ePHI, restricting access to authorized users, such as positioning screens away from public view and implementing automatic screen locks after periods of inactivity.
Device and Media Controls
This standard governs the receipt and removal of hardware and electronic media containing ePHI into and out of a facility, and the movement of these items within the facility. Required implementation specifications include disposal procedures ensuring ePHI is properly destroyed or rendered unreadable, media re-use procedures removing ePHI before equipment reuse, and accountability tracking for hardware and media movements.
Environmental Safeguards
Physical safeguards also extend to environmental protections such as fire suppression systems, climate control for server rooms, and backup power supplies that protect information systems from environmental damage and ensure continued availability during utility disruptions.
Practical Application
HIM departments should participate in facility security assessments, ensure paper records and portable media are stored securely, and enforce clean desk policies in areas where PHI is accessible, reinforcing physical safeguards as a critical layer of the organization's overall security posture.
