Introduction to LOINC
Logical Observation Identifiers Names and Codes, or LOINC, is a standard used to identify laboratory tests, clinical observations, and other measurements. RHIA candidates should understand that LOINC solves a specific interoperability problem: different laboratories and vendors often use different local codes for the same test, making it difficult to compare or aggregate results across systems.
Structure of a LOINC Code
- Component, the substance or entity being measured
- Property, the characteristic being measured, such as mass concentration
- Timing, whether the measurement is a point in time or over an interval
- System, the specimen or sample type
- Scale, whether results are quantitative, ordinal, or nominal
- Method, the technique used when relevant
Together these six axes create a unique, computable identifier for each distinct test or observation.
Why LOINC Matters for HIM
LOINC underpins meaningful use and interoperability requirements, enabling laboratory results to flow accurately between hospitals, reference labs, public health agencies, and health information exchanges. Without a shared code set, a basic metabolic panel from one lab could not be reliably compared to the same panel from another facility.
Mapping Local Codes to LOINC
Most laboratory information systems store local test codes that must be mapped to LOINC for external reporting and exchange. HIM professionals may be involved in validating these mappings, especially when a facility changes laboratory vendors or merges with another organization. Errors in LOINC mapping can cause incorrect results to populate a patient's record or public health report.
Public Health and Analytics Applications
LOINC codes are essential for public health surveillance, such as reportable disease tracking and syndromic surveillance, because they allow health departments to aggregate data from many disparate sources reliably. LOINC is also foundational to clinical analytics projects, including case detection algorithms and quality measure calculations that rely on specific lab values.
Exam Tips
Expect questions distinguishing LOINC from CPT and SNOMED CT. Remember that CPT describes the billable service of performing a test, LOINC identifies the specific test and result, and SNOMED CT can describe the clinical finding associated with an abnormal result. Understanding these distinctions is a frequent RHIA exam theme.
