Understanding Natural Language Processing

Natural language processing, or NLP, is a branch of artificial intelligence focused on enabling computers to interpret, analyze, and derive meaning from human language. In healthcare, NLP addresses the challenge that a substantial portion of clinical information exists as unstructured free text within notes, reports, and narratives.

NLP Applications in Coding

Computer-assisted coding systems use NLP to scan clinical documentation, identify relevant clinical concepts and their context, such as negation or family history versus current diagnosis, and generate suggested codes for coder review. This automation can significantly reduce the manual effort required to identify codable diagnoses and procedures within lengthy documentation.

Clinical Documentation Improvement

NLP tools also support concurrent clinical documentation improvement by scanning documentation in real time to flag potential specificity gaps, such as an unspecified diagnosis that could be further clarified, allowing CDI specialists to generate queries before discharge rather than retrospectively.

Beyond coding, NLP enables extraction of structured data elements from unstructured sources for research, quality reporting, and registry submission, reducing reliance on manual chart abstraction. NLP can also power search functionality that allows clinicians to quickly locate relevant information buried within lengthy patient records.

Computer-Assisted Coding Integration

Modern CAC platforms combine NLP with underlying coding logic and grouper software to present coders with a streamlined workflow, highlighting the specific text supporting each suggested code so coders can efficiently validate accuracy against the source documentation.

Technology Limitations

NLP accuracy varies depending on documentation quality, medical specialty, and the complexity of clinical language, including abbreviations, non-standard terminology, and ambiguous phrasing. NLP systems can misinterpret negated findings, confuse similar-sounding terms, or miss context clues that a human coder would readily catch. Because of these limitations, human review remains essential, and organizations should establish ongoing quality auditing to monitor NLP-driven suggestions against source documentation, ensuring technology augments rather than replaces professional coding judgment.
