The Importance of Patient Safety Analytics

Analyzing patient safety events, including near misses and adverse events, allows healthcare organizations to identify systemic issues before they cause serious harm. RHIA candidates must understand how event data is collected, categorized, and analyzed to support a culture of safety.

Categories of Patient Safety Events

  • Sentinel events, which result in death or serious harm and require immediate investigation.
  • Adverse events, which cause harm but may be less severe than sentinel events.
  • Near misses, where an error occurred but did not reach the patient or cause harm.
  • Hazardous conditions, circumstances with the potential to cause an event even if none has yet occurred.

Data Collection Methods

Most organizations use incident reporting systems that allow staff to voluntarily report events. Analytics teams supplement this with trigger tool methodology, which uses specific clinical clues in the medical record to identify events that may not have been voluntarily reported, since underreporting is a well-documented challenge.

Trending and Root Cause Analysis

Once data is collected, analysts trend events over time by type, location, and severity to identify patterns. A spike in falls on a particular unit, for example, may prompt a root cause analysis, a structured method for identifying the underlying causes of an event rather than just the immediate cause.

  2. Identify contributing factors using tools such as fishbone diagrams.
  3. Determine root causes rather than surface-level symptoms.
  4. Develop and implement corrective action plans.
  5. Monitor data to confirm the corrective action was effective.

Statistical Tools for Safety Trending

Control charts and run charts are commonly used to distinguish between random variation in safety event rates and a true shift requiring intervention. Rate calculations, such as falls per one thousand patient days, allow for fair comparison across units of different sizes.

Regulatory Reporting Requirements

Certain sentinel events must be reported to accrediting bodies such as The Joint Commission, and some states require mandatory reporting of specific adverse events. HIM professionals often manage the data systems that support this reporting and ensure confidentiality protections for peer review data.
