Foundations of Healthcare Research
Healthcare research relies on systematic methods to generate reliable evidence supporting clinical practice and organizational decision-making. HIM professionals often support research activities through data abstraction, data governance, and privacy compliance, making familiarity with basic research methodology valuable for the RHIA exam and professional practice.
Study Designs
Experimental study designs, such as randomized controlled trials, actively assign subjects to intervention and control groups to test causal relationships under controlled conditions. Observational designs, including cohort studies, case-control studies, and cross-sectional studies, examine relationships between variables without researcher-controlled intervention, making them useful when experimental manipulation is impractical or unethical.
Sampling Methods
Probability sampling methods, such as simple random sampling, stratified sampling, and systematic sampling, give every member of the population a known chance of selection, supporting generalizable findings. Non-probability sampling methods, such as convenience sampling, are easier to conduct but carry greater risk of selection bias limiting generalizability.
Variables
Independent variables are the factors researchers manipulate or examine as potential causes, while dependent variables represent the outcomes being measured. Confounding variables are extraneous factors that correlate with both the independent and dependent variable, potentially distorting the apparent relationship between them if not properly controlled through study design or statistical adjustment.
Data Collection Instruments
Research data collection instruments include surveys and questionnaires, structured chart abstraction tools, and direct observation protocols. Instruments should be validated for reliability, meaning they produce consistent results, and validity, meaning they accurately measure the intended construct.

Evidence-based practice integrates the best available research evidence with clinical expertise and patient values to guide healthcare decisions. A thorough literature review, often organized using a hierarchy of evidence that ranks systematic reviews and meta-analyses above individual studies, helps practitioners identify the strongest available evidence to inform practice guidelines and organizational policy decisions.
