The Role of Statistics in Quality Improvement

Quality improvement in healthcare relies heavily on statistical methods to determine whether changes to a process result in measurable, meaningful improvement rather than random variation. RHIA candidates must be comfortable interpreting and applying common statistical tools used in performance improvement projects.

Control Charts

Control charts, also called statistical process control charts, track a process metric over time and distinguish between common cause variation, which is normal and expected, and special cause variation, which signals an unusual event requiring investigation. Upper and lower control limits are typically set at three standard deviations from the mean.

Measures of Central Tendency and Variation

  • Mean, the arithmetic average of a dataset.
  • Median, the middle value, useful when data is skewed by outliers.
  • Mode, the most frequently occurring value.
  • Standard deviation, a measure of how spread out data points are from the mean.

Hypothesis Testing Basics

Quality improvement studies often use hypothesis testing to determine whether an observed difference is statistically significant. A null hypothesis assumes no difference exists, while the alternative hypothesis assumes a real effect. A p-value below the chosen threshold, commonly 0.05, suggests the result is unlikely due to chance alone.

The PDSA Cycle and Data

The Plan-Do-Study-Act cycle is a foundational quality improvement framework. Data is collected during the Do phase and analyzed during the Study phase to determine whether the intervention had the intended effect. Run charts, which plot data points over time without control limits, are frequently used in early PDSA cycles before more formal control charts are applied.

Common Tools in Quality Studies

  1. Pareto charts, which identify the most significant factors contributing to a problem using the eighty-twenty principle.
  2. Fishbone diagrams, which organize potential causes of a problem into categories.
  3. Scatter diagrams, which test for correlation between two variables.
  4. Histograms, which display the frequency distribution of a dataset.

Sample Size and Reliability

Small sample sizes can produce misleading results in quality studies. HIM professionals should understand basic concepts of statistical power and how sample size affects the reliability of conclusions drawn from a quality improvement project.
