Overview of Value-Based Purchasing
The Hospital Value-Based Purchasing Program, or VBP, is a CMS budget-neutral program that adjusts hospital Medicare payments based on quality performance rather than volume alone. Funding for incentive payments comes from a withhold applied to participating hospitals' base operating DRG payments, redistributed based on total performance scores.
VBP Domains
The program evaluates hospitals across several domains: clinical outcomes, which include mortality and complication measures; patient safety; person and community engagement, primarily measured through the HCAHPS patient experience survey; and efficiency and cost reduction, which examines Medicare spending per beneficiary. Each domain carries a specific weight in the final scoring calculation.
Total Performance Score
A hospital's total performance score is calculated by combining domain scores according to their assigned weights. Within each domain, hospitals earn points based on both achievement, comparing performance against a national benchmark, and improvement, comparing performance against the hospital's own prior baseline period, with the higher of the two scores counted for each measure.
Payment Adjustments
Hospitals with total performance scores above the threshold receive a positive payment adjustment, while those below the threshold receive a reduced payment. Because the program is budget neutral, the total amount of incentive payments distributed equals the total amount withheld from participating hospitals, meaning underperformance directly funds the incentive payments earned by higher performers.
Clinical Outcomes Measures
Clinical outcome measures typically include thirty-day mortality rates for conditions such as heart failure, pneumonia, and acute myocardial infarction, along with complication measures such as postoperative complications following elective procedures.
Patient Experience
The HCAHPS survey captures patient perspectives on communication with nurses and doctors, responsiveness of hospital staff, cleanliness and quietness of the environment, and overall hospital rating. HIM professionals support VBP performance by ensuring accurate coding that reflects true patient severity and by contributing to data validation processes that maintain the integrity of publicly reported quality metrics.
