Clinical Terminology and Classification Systems

Clinical terminologies and classification systems form the backbone of health data standardization. They provide the structured vocabulary that enables consistent documentation, accurate coding, meaningful data analysis, and interoperability across healthcare systems. RHIA candidates must understand the purpose, structure, and application of each major system.

Terminology vs. Classification

It is important to distinguish between clinical terminologies and classification systems:

  • Clinical terminology (reference terminology): A comprehensive set of concepts with defined relationships used to capture clinical detail at the point of care. Examples include SNOMED CT and LOINC. Terminologies support granular documentation.
  • Classification system: A system that groups related concepts into categories for statistical reporting, billing, and epidemiological analysis. Examples include ICD-10-CM and ICD-10-PCS. Classifications trade granularity for aggregation.

The two are complementary. Data captured using a reference terminology at the point of care can be mapped to a classification system for secondary purposes like billing and population health reporting.

ICD-10-CM (International Classification of Diseases, 10th Revision, Clinical Modification)

ICD-10-CM is the U.S. clinical modification of the World Health Organization's ICD-10. It is used to code diagnoses in all healthcare settings.

  • Contains approximately 72,000 codes.
  • Codes are alphanumeric, 3 to 7 characters long.
  • Structure: Category (3 characters) + Etiology/Anatomic site/Severity (4th-6th characters) + Extension (7th character for certain chapters).
  • Maintained by the National Center for Health Statistics (NCHS), part of the CDC.
  • Updated annually, effective October 1.

ICD-10-PCS (Procedure Coding System)

ICD-10-PCS is used exclusively for coding inpatient hospital procedures. It replaced ICD-9-CM Volume 3.

  • Contains approximately 78,000 codes.
  • All codes are exactly 7 alphanumeric characters.
  • Each character position has a defined meaning: Section, Body System, Root Operation, Body Part, Approach, Device, and Qualifier.
  • Maintained by CMS.
  • The system is designed to be multiaxial and expandable, meaning new codes can be created by combining existing values without restructuring the system.

CPT (Current Procedural Terminology)

CPT is the primary code set for reporting outpatient procedures and professional services. It is maintained by the American Medical Association (AMA).

  • Category I codes: Five-digit numeric codes for procedures and services (e.g., 99213 for an office visit).
  • Category II codes: Supplemental tracking codes for performance measurement (e.g., 1234F); these are optional.
  • Category III codes: Temporary codes for emerging technology and procedures (e.g., 0001T).
  • CPT is updated annually, effective January 1.

HCPCS (Healthcare Common Procedure Coding System)

HCPCS has two levels:

  • Level I: CPT codes (described above).
  • Level II: Alphanumeric codes maintained by CMS for items and services not covered by CPT, such as durable medical equipment, prosthetics, orthotics, ambulance services, and certain drugs. Codes begin with a letter (A-V) followed by four digits.

SNOMED CT (Systematized Nomenclature of Medicine - Clinical Terms)

SNOMED CT is the most comprehensive clinical terminology in healthcare. It is maintained by SNOMED International and distributed in the U.S. by the National Library of Medicine.

  • Contains over 350,000 concepts organized into hierarchies.
  • Each concept has a unique numeric identifier, fully specified name, and preferred term.
  • Concepts are linked by defined relationships (e.g., "is-a," "finding site," "causative agent").
  • Used in EHRs to capture clinical detail at the point of care.
  • Required for problem lists, encounter diagnoses, and procedures in certified EHR technology under the Promoting Interoperability program.

LOINC (Logical Observation Identifiers Names and Codes)

LOINC provides standard codes for laboratory tests, clinical observations, and survey instruments. It is maintained by the Regenstrief Institute.

  • Each LOINC code identifies a specific observation based on six axes: Component (analyte), Property, Timing, System (specimen), Scale, and Method.
  • Widely used for electronic reporting of lab results and for interoperability between laboratory information systems and EHRs.
  • Required for lab result reporting under Promoting Interoperability.

Mapping Between Systems

Mapping is the process of linking concepts from one terminology or classification to another. For example, SNOMED CT concepts are mapped to ICD-10-CM codes to support billing workflows. The General Equivalence Mappings (GEMs) were developed to facilitate the transition between ICD-9-CM and ICD-10-CM/PCS. Mapping requires careful maintenance to ensure clinical accuracy and should be validated by subject matter experts.

Exam Preparation Tips

RHIA exam questions frequently test your ability to identify which code set is used in a given scenario (inpatient procedures use ICD-10-PCS, outpatient procedures use CPT, diagnoses in all settings use ICD-10-CM). Know the maintaining organizations and update cycles. Understand the difference between a terminology and a classification, and be prepared to explain why SNOMED CT and LOINC are important for EHR interoperability.
